Medicare sponsors a chronic care management program... Do you qualify?

Have you heard? ClareMedica is offering a new program for Medicare patients called Chronic Care Management, or CCM. The program was designed by Medicare to give doctors the tools and the resources to offer faster, more direct, and more frequent care to their patients with multiple, long lasting health conditions. Examples of those conditions are high blood pressure, diabetes, arthritis, cancer, stroke, and obesity, to name only a few. If you have at least two of those conditions, you might be eligible for CCM. If you are, we believe you would benefit from this program and we encourage you to participate.

Benefits of the program include a monthly call from one of our care navigators to check in with you and, if you need it, help with medication refills, making appointments, and staying in touch with your doctor. Your navigator will also work with you to develop and follow a personalized care plan designed specifically for you.
